• Health Record Information

• 18 months of age and older are required to have chickenpox vaccine (varicella or Varivax) or a reliable history of chickenpox disease.
• Preschool, needs to have the 2nd Chicken pox vaccine and 2nd MMR.

• Chickenpox vaccine (varicella or Varivax) or a reliable history of chickenpox disease.
• Second MMR (Measles, Mumps and Rubella)
• 5th DTaP (Diphtheria, Tetanus, acellular Pertussis) (unless #4 on or after 4th birthday)
• 4th Polio (unless #3 on or after 4th birthday)
• Hepatitis B series (series of 3 shots usually given soon after birth)

• 1^st, 2^nd and 3^rd graders are required to have Hepatitis B series.

• 6^th and 7^th graders need to have TDAP.
• 7^th and 8^th graders are required to have Hepatitis B series and their second MMR.

A current “Certificate of Immunization” needs to be on file
in the school office for each student.  Forms can be ordered
from our school.  Childhood immunizations are available at the
child's physician or health care provider.  In Pueblo, they are
available at the Wellness Center or at the
Pueblo City County Health Department at (719) 583-4373
