Health Record Information

Preschool:
  • 18 months of age and older are required to have chickenpox vaccine (varicella or Varivax) or a reliable history of chickenpox disease.
  • Preschool, needs to have the 2nd Chicken pox vaccine and 2nd MMR.
Kindergarten:
  • Chickenpox vaccine (varicella or Varivax) or a reliable history of chickenpox disease.
  • Second MMR (Measles, Mumps and Rubella)
  • 5th DTaP (Diphtheria, Tetanus, acellular Pertussis) (unless #4 on or after 4th birthday)
  • 4th Polio (unless #3 on or after 4th birthday)
  • Hepatitis B series (series of 3 shots usually given soon after birth)
Elementary School:
  • 1st, 2nd and 3rd graders are required to have Hepatitis B series.
Middle School:
  • 6th and 7th graders need to have TDAP.
  • 7th and 8th graders are required to have Hepatitis B series and their second MMR.

Medical, religious & personal exemptions continue to apply

A current “Certificate of Immunization” needs to be on file in the school office for each student. Forms can be ordered from our school. Childhood immunizations are available at the child's physician or health care provider.
